First Time Clients
Please be aware that you are required to fill out and complete one (or more) medical forms. These forms MUST be completed, in-full, to the best of the client's knowledge PRIOR to receiving any bodywork.
The therapist will review these forms with you, and may ask general health questions about your current and past medical history. It is very important that you inform the therapist of ALL health problems (current and past) and ANY medications that you are taking. If you are under a doctor's care, it is strongly advised that you receive a written recommendation for massage or bodywork prior to any session. Depending on the condition, approval from your doctor may be required.
For your convenience, required forms are listed below. Please bring the necessary completed forms with you to your appointment.
Please arrive 15 minutes prior to your appointment time so that the therapist can review these forms with you.
